Following our discussion an appointment has been made for _______________________ to see

Name:

Centre:

Telephone No:

The following notes have been provided to ensure the referral runs as smoothly as possible.

The appointment has been arranged because we felt that it would be helpful to have a further opinion regarding this condition from someone with in depth experience of the condition.

Treatment may also involve specialised expertise and/or equipment which may not be available at the practice.

Attending for your appointment:

  • Endeavour not to be late.
  • If there is likely to be an unavoidable delay please telephone the referral centre at the number above.
  • Be prepared to settle the cost at the time of the consultation. We have given some indication of cost but if you wish for more precise details please do not hesitate to telephone the referral centre for advice.
  • If your pet is insured take all the details with you. Payment is still likely to be expected at the time but usually cheques or credit cards are accepted. However if in doubt, please telephone first.

What happens after the appointment?

The consultant will explain the procedure and feel free to discuss estimated cost and probable outcome if you are concerned.

After the examination and consultation we will receive a report and opinion from the consulting veterinary surgeon who will also give you some indication of when to call in to see us. Again if in doubt, please do not hesitate to enquire.

If you have any further concerns and feel we can help further with this matter please feel free to call us. We will be pleased to help.

We hope all goes well.
