If you are wishing to register and book an appointment, please make use of our online booking facility. If you want to register your pet(s), but do not need to make an appointment yet, you can use this form.  Alternatively you can pop in or give us a call on 01803 606059 (Torquay) or 843836 (Paignton).

Please use this form to register yourself and your pets with our surgery.
  • Your full postal address, including postcode
  • If you don't have a landline, please enter your mobile number here.
    Please tick which of the above you are happy to receive; you can easily amend this at a later date.
    Please tick to confirm you have viewed our Privacy Notice
  • Please enter the following for each pet you wish to register: Name, Species e.g. dog or cat, Breed, Age, and Gender (including whether neutered or not).
  • Please give details of where your pet is currently registered, so that we can request their medical notes. If they are not registered anywhere, please write N/A.
